This policy describes how Tree of Life Acupuncture PC collects, uses, and shares consumer health data through our website, as required by the Washington My Health My Data Act (RCW 19.373). It applies to Washington State residents who visit our website or submit information through our online forms.

What Consumer Health Data We Collect

When you use our website contact, consultation request, or appointment scheduling forms, we may collect:

●Date of birth

●Whether you are a new or returning patient

●Responses to open-ended questions you choose to share (for example, questions about your fertility health goals or reason for your visit)

We also use Google Analytics 4 (GA4) and Google Tag Manager to understand how visitors use our website. This data is aggregated and anonymized — it tells us things like which pages are visited most often, not who visited them.

How We Use This Data

We use the information collected through our website forms to:

●Respond to your consultation or appointment requests

●Contact you to schedule or confirm visits

●Improve how our website works and how we communicate with prospective patients

We use GA4 data strictly to analyze overall website traffic patterns. We do not sell your health data to anyone. We do not use your health data for targeted advertising.

Who We Share It With

Google receives anonymized, aggregated analytics data through GA4 and Google Tag Manager. Google does not receive your name, contact information, or any individually identifiable health data from us through these tools.

We do not share your consumer health data with any other third parties for health data purposes.

A note about your medical records: If you become a patient, your protected health information (PHI) is stored and managed through our electronic health record system, AdvancedMD. That data is governed separately by our HIPAA Notice of Privacy Practices, not by this policy.

Your Rights Under Washington Law

The Washington My Health My Data Act gives you the following rights:

●Access — You may request a copy of the consumer health data we have collected about you.

●Deletion — You may request that we delete your consumer health data.

●Withdrawal of consent — You may withdraw consent for us to collect or use your consumer health data going forward.

We will respond to verified requests within 45 days. In limited circumstances where more time is needed, we may extend this period and will notify you of the reason.
